feat: session rename — display name tracking

Add a display name field to the sessions table and a PATCH /session/ endpoint to update it. When an agent framework renames a session (e.g. Claude Desktop auto-titles), that name should be captured and stored. The display name is separate from the GUID and is purely for human readability in Ragger tooling.
