Migrate remaining MUI usage in template create/editor flows to shared shadcn components (`FormField`, `Input`, `Textarea`, `Checkbox`, `RadioGroup`, `Link`). Also drops `disablePortal` on the publish-version help popover so it is no longer clipped by the dialog overflow.
