Keep the inspector out of production builds and only open project files

`isDevMode()` is a function call, so the bundler couldn't drop the dynamic
import and `ng build` still emitted the inspector as a lazy chunk (never
loaded, but shipped). Checking `ngDevMode` directly lets production builds
remove it entirely.

The path sent to `/__open-in-editor` now has to be project-relative (no
absolute paths or `..`) and come from an element rendered by an Angular
component, so injected markup can't be used to open arbitrary files.

Also list the known limitations in the README: Shadow DOM, paths with a custom
rootDir or in multi-project workspaces, and the Alt key on some platforms.
