Should close issue #336 mashlib v2.2.0 not rendered correctly
Updated index.js to make databrowser boot more reliable and keep CDN/local behavior aligned.
CDN mode now loads the matching mashlib shell template from the selected version, applies its title/body, and then loads CSS/JS with explicit error handling.
Also added a logout refresh fallback (with one-shot guard and URL param cleanup) in both CDN and local modes to fix stale UI state after logout.
