  1. Clean up how line numbers are computed in tests 

    VS Code/LSP is zero-based, but DAP is one-based. There were a lot of +1's around the place and comments explaining why. This wraps them all up in a single `dapLineOf()` helper and uses `^` on the end of a `BREAKPOINT` marker as a signal that the breakpoint goes on the line above.
