Most AI reviewers post 20 to 40 comments on a single pull request. After a week, the team mutes the bot. The fix is not better prompts. It is a higher bar for what becomes a comment. Instalog reviews every pull request, runs each finding through Semgrep and codebase context, validates with a second model, and posts only what survives. Default cap is five comments per PR. The bar for posting is acceptance, not coverage.
